Terms of Agreement for Monthly EFT Donations
I authorize KCUR to deduct the above indicated amount each month from my checking account on the 5th day of each month or equivalent bank business day. I understand:
- This authorization is ongoing, automatically renewing from year-to-year, and will remain in effect until I notify KCUR in writing.
- Monthly transfers will occur on the 5th day of each month or equivalent bank business day. My bank statements will reflect these transfers.
-For tax purposes, I will receive a summary of my donations at the end of each calendar year.
